Require at least Qt 5.4.0

While I am not aware that anything actually needs 5.4.0, sweeper
currently depends on KF 5.23.0 which requires 5.4.0 rendering a
lower requirement moot.
Also use that version in -DQT_DISABLE_DEPRECATED_BEFORE to avoid
build errors with newer Qts.
