Adjust to the string changes in D26551 and improve the presentation
overall to make it more likely that people will feel comfortable
using it and submitting information.

This is the header/subtitle style we have to use because @mart and @broulik veto bold type in titles. :) Simply using slightly larger text in the header isn't good enough and results in a muddy appearance.

Open to suggestions.
